Substance Use Disorder Program Coordinator

Squirrel Hill Health Center is hiring a Substance Use Disorder (SUD) Program Coordinator to support recovery, improve health outcomes, and sustain public health initiatives like harm reduction and medication-assisted treatment (MAT). This role serves as a liaison between patients, providers, staff, and community partners, while coordinating all aspects of the SUD program.

Responsibilities:

Coordinate the SUD/MAT program and track patient progress.
Conduct patient assessments, create care plans, and provide education on harm reduction and overdose prevention.
Facilitate referrals to behavioral health, counseling, and social services.
Maintains program standards of COE, MAT and SUD programming, including workflows and documentation for providers and patients.
Collects, tracks, and reports program data in REDCap and other reporting systems. Prepares reports for MCOs, PittPERU, funders, and accrediting agencies.
Support Mobile Unit visits, including check in and appointment scheduling.
Build relationships with external providers and agencies to support patient care.

Qualifications:

Bachelor’s degree in social work, public health, counseling, nursing, or related field (MSW preferred).
Experience in substance use disorders or behavioral health preferred.
Knowledge of MAT, harm reduction, medical terminology, and EHR systems.
Strong organizational, communication, and documentation skills.
Proficiency in Microsoft Office/Google Suite; REDCap experience preferred.
Bilingual ability is a plus.
